This position, an employee of the Nassau County Health Department (NCHD), works to protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts. This highly responsible position is part of the Community Health Promotions Team that works in the community to plan and conduct programs and initiatives to improve public health. This position promotes, maintains, and improves individual and/or community health by assisting individuals and communities to adopt healthy behaviors through education and/or awareness campaigns.

 

 

Your Specific Responsibilities:

  • Interviewing potentially eligible clients via telephone and in person.
  • Accurately and appropriately entering all WIC information into FL-Wise.
  • Ensures duplicate clients are not created and if a duplicate is determined, follows procedures to get duplicate removed.
  • Schedules clients with the appropriate appointment type for all eligible members of a family.
  • Perform WIC benefits issuance.
  • Orientates new clients to WIC providing information on rights and obligations, use of WIC EBT card, policies and procedures, and WIC clinic operations.
  • Schedules return appointments.
  • Informs the clients of their need for re-certification information when required according to the program guidelines.
  • Performs administrative site duties.
  • Answers telephone in a timely manner and returns calls.
  • Maintains accurate inventories, including special formula and WIC EBT cards.
  • Issues breast pumps according to local WIC protocol and re-evaluates loaned double pumps monthly according to policy.
  • Investigates and documents incidents and complaints concerning clients and vendors. Works reports as assigned by supervisors.
  • Verify WIC eligibility.
  • Interview clients to determine income eligibility at certification and/or re-certification based on established federal guidelines.
  • Performs adjunctive eligibility where applicable (use FLMMIS) and records in computer.
  • Accurately updates temporary certifications (short certifications) in computer, completes forms, and scans forms into system.
  • Investigates and documents incidents and complaints concerning clients and vendors.
  • Works reports as assigned by supervisors.
  • Obtains anthropometric and hematological data from clients as needed for all appointment types.
  • Obtains immunization data and reviews for accuracy.
  • Refers clients to other social services as requested.
  • Codes services in FL-Wise and HMS EARs accurately.
  • Participates in outreach events and QI projects
  • Performs all other duties as assigned.

Required Knowledge, Skills, and Abilities:

  1. Ability to use Microsoft applications to analyze data and create reports.
  2. Knowledgeable of Health Department policies and procedures.
  3. Knowledge of the Community Health improvement framework and processes.
  4. Knowledge of Community Health Assessment and Prioritization processes.
  5. Ability to lead, develop, and support group projects.
  6. Skills at identifying leads from external agencies and engaging community partners to achieve a common goal.

 

Qualifications:

Minimum-

  • At least one year experience providing health education services.
  • At least 1 year experience developing educational materials and conducting workshops and or community presentations.
  • Intermediate experience with Microsoft Office.

Preferred-

  • A bachelor's and/or master’s degree in public health (MPH), Health Education, or a related field.
  • Certified Health Education Specialist (CHES) or Certified in Public Health (CPH)
  • Experience working with diverse populations and in community-based settings.
  • Experience in program planning, implementation, and evaluation.
  • Understanding of public health issues, health gaps, and factors impacting of health.
  • Knowledge and familiarity of health promotion strategies.
  • Ability to analyze data and use evidence-based practices to design and implement interventions.
  • Ability to develop educational materials and conduct training or workshops.
  • Familiarity with health assessment tools and techniques.
  • Strong collaboration skills to work effectively with public health agencies, community organizations, and stakeholders.
