Job Summary • Utilizes critical thinking and clinical judgment to support the case management process • Facilitates appropriate healthcare outcomes by coordinating services, scheduling appointments, and assisting members in accessing benefits and resources • Employs care management tools to deliver high-quality service and effective coordination Key Responsibilities • Assist with scheduling healthcare appointments for members • Identify member needs and support access to services and education • Collaborate with team members and providers to support care plans • Use care management systems and tools to document and track activities • Promote effective healthcare navigation and service utilization • Work independently while maintaining virtual collaboration with team members Required Qualifications • Minimum 2 years of experience in behavioral health, social services, or a related field aligned with program focus • Proficiency in MS Word, Excel, Outlook, and PowerPoint • Ability to navigate multiple systems with strong computer and keyboarding skills • Strong analytical, problem-solving, and organizational skills • Excellent written and verbal communication • Ability to work independently and manage multiple priorities Preferred Qualifications • Experience in case management or discharge planning • Managed care experience • Confidence in remote work environments and virtual collaboration tools Compunnel
