Hiawatha HomeCare has an immediate opportunity for a caring nurse to provide 1:1 care for our client in Menomonie, WI!

*SCHEDULE *
* Flexible - choose your own hours!

*CLIENT SPECIFICS*
* 17 years old
* Spastic Quadriplegic Cerebral Palsy
* G-Tube
* Seizures

*SALARY RANGE *
* LPN: $20.00 to $21.33 per hour (DOE)
* RN: $28.00 to $29.50 per hour (DOE)

*CORE RESPONSIBILITIES*
* Provides home care nursing in accordance with the physician's plan of care.
* Provides advice & support to the client's family.
* Reports significant changes in the client's condition to the family, RN Clinical Manager, & physician.
* Obtains physician orders for changing client care needs.
* Maintains accurate & current plan of care, physician's orders, and medication profile for the client.
* Charts cares thoroughly & accurately during shift.
