The UW MEDICAL CENTER - MONTLAKE PHARMACY DEPARTMENT is seeking a highly-motivated CLINICAL PHARMACIST – HEMATOLOGY/ONCOLOGY.
WORK SCHEDULE
- 40 Hours Per Week
- Rotating/Variable Shift
POSITION HIGHLIGHTS
- Provide pharmaceutical care for our inpatient oncology patient population (including Hematology/Oncology and Bone Marrow Transplant).
- Be a vital part of the inpatient multidisciplinary patient care team.
PRIMARY JOB RESPONSIBILITIES
- Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
- Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
- Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.
- Provide drug information and education to medical teams, nursing, and dietitian staff.
- Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders at each visit.
- Provide primary preceptorship of pharmacy residents and student pharmacists.
- Perform duties of a Pharmacist 2.
Strong candidates will have completed an ASHP-accredited PGY2 residency in oncology pharmacy practice.
REQUIRED POSITION QUALIFICATIONS
- Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.
- State of Washington Pharmacist License.
